18-39 years old – $100/month.

40 years and older – $120/month.

Special Snowbird Rate (for Non-Medicare Patients ONLY) – $150/month for 6 months.

Once a year comprehensive Annual with 1 Follow Up to review results – $250.

One time Acute Urgent Visit – $150. Follow up for Acute Urgent Visit – $75.
